Entrusted Heart Homecare is a nurse-owned and nurse-led home care agency dedicated to helping older adults, veterans, individuals recovering from illness or injury, and people with disabilities remain safe and independent at home. We provide compassionate, dependable support tailored to each client’s individual needs. Our core services include personal care, companionship, assistance with activities of daily living, meal preparation, medication reminders, light housekeeping, transportation, respite care, and post-discharge support.

How long have you been in business?

Entrusted Heart Homecare was established in 2022 and has been serving individuals and families for approximately 4 years.

Where are you located, and what’s your service area?

Our Alabama operations serve Montgomery, Tuskegee, and surrounding communities throughout Montgomery and Macon counties. Entrusted Heart Homecare also maintains a licensed home care operation in California.

How many employees do you have?

Entrusted Heart Homecare maintains a lean and flexible staffing structure, allowing us to grow our team responsibly based on client needs. We maintain a network of qualified caregivers and nursing professionals who share our commitment to providing compassionate, dependable care. We continue to grow our team as we expand our services and accept additional referrals.

Toni Norman, BSN, RN — Owner and Administrator
Toni founded Entrusted Heart Homecare and brings approximately 18 years of nursing experience to the organization. She oversees client care, operations, caregiver standards, and community partnerships. Her clinical background supports the agency’s commitment to providing safe, compassionate, and dependable care.

How long has your organization been part of the Watershed network, and how are you currently using Watershed?

Entrusted Heart Homecare joined the Watershed network in 2025. We use Watershed to strengthen our connections with healthcare and community partners, increase awareness of our services, and receive appropriate referrals for individuals who need assistance at home. The platform supports our goal of improving care coordination and helping clients transition safely from healthcare settings back into the community.

Does your team have any favorite features of the Watershed platform and/or community?

We especially value Watershed’s ability to connect healthcare providers and community-based organizations in one collaborative network. The referral platform makes it easier for organizations to identify available resources, communicate across the care continuum, and connect individuals with the support they need. We also appreciate the opportunity to build meaningful partnerships with other organizations serving our community.

What do you think are the biggest challenges on the horizon for the healthcare industry?

Some of the greatest challenges will be meeting the needs of a growing older-adult population, addressing caregiver and healthcare workforce shortages, and ensuring that individuals can access affordable, dependable care in their own communities. Preventing gaps in care after hospital discharge will also remain important. Strong collaboration among hospitals, healthcare professionals, home care agencies, and community organizations will be essential to improving outcomes and reducing avoidable hospital readmissions.
